摘 要 目的:探讨尿微量白蛋白和尿糖检测对糖尿病早期肾损伤的临床意义.方法:选择2014年4月—2016年4月收治的糖尿病早期肾损伤患者80例为观察组,并同时收集体检健康者75人作为对照组,进行尿糖和尿微量白蛋白检测,并对检测结果进行比较.结果:观察组尿糖阳性65例(81.25%),尿微量白蛋白阳性45例(56.25%);对照组则分别为4例(5.33%)和1例(1.33%),组间差异均有统计学意义(P<0.05).观察组尿微量白蛋白和尿糖含量分别为(42.09±7.23)mg/L和(1.88±0.71)mmol/L,对照组则分别为(13.55±3.91)mg/L和(1.01±0.35)mmol/L,组间差异有统计学意义(P<0.05).结论:检测糖尿病患者的尿糖与尿微量白蛋白含量对判断早期肾损伤有重要意义,有助于肾损伤的早发现和早治疗.

Urine albumin and urine glucose detection on the clinical significance of early renal injury in diabetes mellitus

ABSTRACT Objective: To investigate the urinary albumin and urine sugar detection on the clinical significance of early renal damage in diabetes. Methods: From April 2014 to April 2016 80 cases of early diabetic kidney injury were selected as an observation group, and at the same time, 75 healthy people were collected as a control group, urine glucose and urine albumin were detected, and the detection results were compared. Results: The observation group had 65 cases (81.25%) of positive urine, and 45 cases (56.25%) of positive urinary albumin, the control group had 4 cases (5.33%) and 1 cases (1.33%), respectively, and the difference between the two groups was statistically significant(P<0.05). Urinary albumin and urine sugar content of the observation group were (42.09±7.23) mg/L and (1.88±0.71) mmol/L, respectively, those of the control group were (13.55±3.91) mg/L and (1.01±0.35) mmol/L, and the difference between the two groups was statistically significant (P<0.05). Conclusion: Detection of urine glucose and urinary albumin content in the patients with diabetes to determine the early renal injury has important significance, and is helpful for early detection and early treatment of kidney injury.

糖尿病是因为胰岛素无法发挥作用或胰岛素分泌缺陷而导致以高血糖为主要特征的代谢性疾病,常见症状有多尿、多飲、多食、无力等,患者往往容易出现代谢功能紊乱和免疫力降低,从而诱发诸多并发症,如糖尿病视网膜病变、糖尿病足、糖尿病肾病等,其中肾损伤是较为常见的并发症[1].糖尿病早期肾损伤在临床上缺乏典型症状,不容易被患者察觉,且常规检查方法准确率不高,容易延误治疗时机,加重患者病情,所以早发现、早诊断尤为重要.本文探讨尿微量白蛋白和尿糖检测对糖尿病早期肾损伤的临床价值.

1 资料和方法

1.1 一般资料

选择2014年4月—2016年4月新余新钢中心医院收治的80例糖尿病早期肾损伤患者为观察组,其中女性35例,男性40例,病程1~14年,平均(7.2±3.6)年.选择同期体检健康者75人为对照组,其中女性32人,男性48人,年龄27~78岁,平均(50.5±16.3)岁.

1.2 方法

收集两组尿液进行尿糖和尿微量白蛋白检测.①尿糖检测采用干化学试剂带葡萄糖氧化酶法.②尿微量白蛋白检测:尿液采集前,受试者保持8 h以上空腹,按照常规方法用容器收集尿液10~20 ml,对中段尿离心后取上清液10 μml检测微量白蛋白含量.根据尿糖标准,尿微量白蛋白<20 mg/L,尿糖值为1.67~2.77 mmol/L判断为阴性,反之则为阳性[2].
