[摘 要] 目的 探讨磁共振成像(Magnetic Resonance Imaging,MRI)在卵巢癌的定性诊断及临床分期中的应用价值.方法该研究方便选取于2015年1月—2017年1月對该院接受诊断与治疗的46例卵巢肿瘤患者进行磁共振成像检查,并将患者的磁共振成像检查结果与病理诊断结果进行对比分析. 结果 MRI检查与病理诊断结果的符合情况,吻合系数kappa为0.663,这表明MRI检查与病理诊断结果的吻合程度较强,且差异有统计学意义(P<0.05).这表明MRI检查与病理诊断在卵巢肿瘤诊断中差异无统计学意义(P=0.072).MRI分期情况为:I期、II期、III期、IV期分别为2例、3例、6例、1例;病理分期情况为:I期、II期、III期、IV期分别为2例、4例、5例、1例;MRI分期与病理分期相比较分期符合率为91.67%(11/12).结论 在卵巢癌的定性诊断中,MRI具有组织分辨力,多参数、多平面成像等优点,诊断准确性较高,能够清晰显示卵巢肿瘤形态学特征及血供特征,可以对卵巢癌的分期情况进行初步确定.

[Abstract] Objective This paper tries to investigate the value of magnetic resonance imaging in the qualitative diagnosis and clinical staging of ovarian cancer. Methods From January 2015 to January 2017, convenient selected 46 cases of ovarian cancer patients in this hospital who accepted the diagnosis and treatment were given the magnetic resonance imaging examination. The magnetic resonance imaging examination results and pathological diagnosis results were compared and analyzed. Results The coincidence of MRI examination and pathological diagnosis showed that the coincidence coefficient kappa was 0.663, which indicated that the MRI examination and the pathological diagnosis result were in good agreement, and the difference was statistically significant(P<0.05). In addition, the McNemar test results showed that P=0.072, which indicated that there was no difference between MRI and pathology in the diagnosis of ovarian tumors. MRI staging: stage I, stage II, stage III and stage IV of 2 cases, 3 cases, 6 cases, 1 case respectively; pathological staging: phase I and phase II, phase III and phase IV of 2 cases, 4 cases, 5 cases, 1 case respectively; the coincidence rate of MRI staging and pathological staging was 91.67%(11/12). Conclusion In the diagnosis of ovarian cancer, MRI possesses resolution, multi parameter and multi planar imaging, with high diagnostic accuracy, which can clearly show the characteristics of the blood supply characteristics and morphology of ovarian tumors, and make preliminarily determination of the staging of ovarian cancer.

在临床上,卵巢肿瘤的发病率较高,是女性生殖系统常见病之一,具体可以分为良性肿瘤及恶性肿瘤[1].其中卵巢癌患者5年存活率仅仅为30%左右,是造成妇科肿瘤死亡率最高的肿瘤类型,所以会对女性的生命安全与身心健康造成严重影响[2-4].近年来随着临床诊疗技术的不断提高,磁共振成像在女性卵巢检查中应用越来越广,可以对患者的卵巢解剖结构进行全面的检查,从而达到早发现、早诊断、早治疗的效果[5].该研究于2016年1月—2017年1月对该院接受诊断与治疗的46例卵巢肿瘤患者采用磁共振成像进行诊断与分期分析,现报道如下.
