[摘 要] 目的 探讨甘精胰岛素联合瑞格列奈治疗2型糖尿病临床疗效.方法 对2016年8月—2017年6月期间在该院接受治疗的58例2型糖尿病患者作为研究对象,两组患者原降糖类药物均停用,给予他们适宜饮食、运动教育与指导,两组严格执行要求并保持一致.在此基础上,对照组患者采取“诺和灵30R”治疗方案,观察组患者采取“甘精胰岛素联合瑞格列奈”治疗方案.监控两组患者餐后2 h血糖（2 hPG）、空腹血糖（FPG）,依据2 hPG、FPG血糖值对胰岛素或瑞格列奈用量进行调整.结果 两组相较而言,治疗后2组2 hPG、FPG、HbAlc都有所下降（P<0.05）,观察组2 hPG、FPG、HbAlc下降程度更为明显（P<0.05）.两组相较而言,观察组治疗前后疗效更为显著（P<0.05）.2组患者均产生了不良反应现象,对照组产生10例（34.5%）低血糖患者,观察组产生1例（3.4%）低血糖,两组相较而言,观察组不良反应发生率较低（P<0.05）.结论 甘精胰岛素联合瑞格列奈治疗2型糖尿病,降糖临床效果较显著,因此,在临床中能够大力实践、推广.

[Abstract] Objective To study the clinical curative effect of insulin glargine combined with Repaglinide in treatment of type 2 diabetes. Methods 58 cases of patients with type 2 diabetes admitted and treated in our hospital from August 2016 to June 2017 were selected as the research objects and both groups stopped the primary hypoglycemic drugs, and the patients were given proper dieting, exercise education and guidance, and both groups were consistent with each other and strictly carried out the above, the control group used the “Novolin 30R” treatment plan, while the observation group used the insulin glargine combined with repaglinide, and the 2 hPG and FPG of the two groups were monitored, and the dosage of insulin or repaglinide was adjusted according to the 2 hPG and FPG blood glucose values. Results The 2 hPG,FPG,HbAlc after treatment of the two groups decreased（P<0.05）, and the decrease degrees of 2 hPG,FPG,HbAlc in the observation group were more obvious（P<0.05）, and the curative effect of the observation group before and after treatment was more obvious（P<0.05）, both groups had adverse reactions, the difference in the incidence rate of hypoglycemia between the control group and the observation group was statistically significant（10 cases vs 1 case, 34.5% vs 3.4%）（P<0.05）. Conclusion The hypoglycemic clinical effect of insulin glargine combined with repaglinide in treatment of type 2 diabetes is obvious, therefore, it can be greatly promoted and applied in clinic.

近年來,甘精胰岛素以注射次数少,吸收稳定,低血糖发生率低及对胰岛β细胞改善功效受到广大患者青睐[1].瑞格列奈可有效刺激人体胰岛素分泌,起效时间较快,疗效作用较长,与甘精胰岛素联合使用,降糖疗效会更显显著,且预防并发症的产生[2].笔者对2016年8月—2017年6月期间在该院接受治疗的58例2型糖尿病患者研究,探讨甘精胰岛素联合瑞格列奈治疗2型糖尿病临床疗效.
1 资料与方法
1.1 一般资料
选取在该院接受治疗的58例2型糖尿病患者作为研究对象.按照随机、均衡理念,对照组患者男16 例,女13例；年龄32～73岁,平均年龄（50.67±2.31）岁；病程2～17年,平均病程（8.3±2.7）年.观察组男15例,女14例；年龄34～75岁,平均年龄（56.34±4.67）岁.病程4～16年,平均病程（9.6±2.5）年.比较两组患者的临床个人资料,差异无统计学意义（P>0.05）.纳入标准：①患者空腹血糖（FPG）<7.0 mmol/L,7.5%>糖化血红蛋白（HbA1c）<10.5%；②嚼服降糖药物≥3个月,且降糖疗效不显著；③所有患者均已签订同意书[3].
