【摘 要】目的:探讨手法对神经病理性疼痛模型大鼠局部镇痛作用效果及其作用机制,为手法镇痛提供理论依据.方法:将32只SPF级雌性SD大鼠随机分为假手术组、模型组、针刺组、手法组,每组

8只.除假手术组外,其他各组建立坐骨神经慢性缩窄性损伤(CCI)大鼠模型.建模成功后,针刺组给予针刺治疗,手法组给予手法治疗.观察手法对大鼠疼痛阈值的影响及手法治疗后施术局部β-内啡肽的含量.结果:大鼠造模后,表现出跛行、抬足、舔足等痛觉过敏表现,和假手术组比较,差异有统计学意义

(P < 0.05).手法能明显提高神经病理性疼痛大鼠的疼痛阈值(P < 0.05);给予手术大鼠手法治疗后,针刺组和手法组大鼠病灶局部β-内啡肽含量明显升高,与假手术组及模型组比较,差异有统计学意义(P < 0.05).

手法组和针刺组比较,差异无统计学意义(P > 0.05).结论:手法对CCI模型大鼠局部镇痛效果显著,其作用机制可能是通过升高施术局部β-内啡肽的含量而达到镇痛效果.

Experimental Study on the Effect and Mechanism of Manipulation on CCI Model in Rats

【ABSTRACT】Objective:To explore the effect of manipulation on local analgesic effect in neuropathic pain rats and its mechanism,provide a theoretical basis for the technique of analgesia.Methods:Thirty-two healthy SD female rats were randomly divided into sham operation group,model group,acupuncture group and manipulation group,with 8 rats in each group.In addition to the sham operation group,the other groups were established sciatic nerve chronic constriction injury(CCI)rat model.After successful modeling,the acupuncture group was given acupuncture treatment,the manipulation group was given manipulation treatment.The content of the local application of beta endorphin manipulation effect on pain threshold in rats and after manipulation treatment.Results:After modeling,the rats showed a limp,lift the foot,foot licking hyperalgesia,compared with sham operation group,the difference was statistically significant(P < 0.05).Manipulation can significantly improve the pain threshold of neuropathic pain rats(P < 0.05).After the surgical rats were given treatment of manipulation,β-endophine content of acupuncture group rats and manipulation group significantly increased,there were significant differences compared to the sham operation group as well as the model group(P < 0.05).There were no significant differences between acupuncture group rats and manipulation group(P > 0.05).Conclusion:The effect of manipulation on local analgesia in CCI model rats was significant,its mechanism may be through increased application of local beta endorphin content and analgesic effect.

疼痛是危及人类健康和影响生活质量的重大医学难题.治疗慢性疼痛的主要困难就是明确其病因、正确选择镇痛方法.手法在缓解经筋系统疼痛方面疗效肯定,具有简便验廉、副作用少的优势.临床工作中,笔者以中医经筋理论为指导,对患者施以手法治疗可以缓解大部分慢性经筋疼痛.但是长期以来,关于手法缓解疼痛的作用机制研究相对较少.已有研究表明,在患者局部施以针刺和手法治疗,能够增加血液中β-内啡肽(β-EP)等具有镇痛作用的神经递质,减少P物质(SP)等具有致痛物质的产生[1-2].β-EP作为一种内源性类 样物质,在抑制疼痛方面起到了重要作用.本实验拟在疼痛部位局部施以适当手法以观察其镇痛效果及其对作用局部β-EP含量的影响,探讨手法局部镇痛的机制,为中医骨伤科手法治疗经筋疾病提供科学依据和理论指导.
